By using our web site, you agree to the following terms of use, which constitute an agreement between Health Care Excel and you. We may occasionally change these terms, but you may view the most current version by clicking on a link at the bottom of any page on our site.
 

Our Information


Our web site includes a wealth of information. Information on the web site is intended for adult health care consumers or other state holders. For health care professionals, it should not serve as a substitute for clinical judgment. Health care consumers should not rely on the information as professional medical advice or use it to replace any relationship with their physician or other qualified health care professional.
 

Information Collection and Use


Personal or other identifying information is not necessary to use our web site. Identifying information generally is required only when users request  materials or register for continuing education activities. The information is voluntarily and only sufficient information will be collected to comply with a request; e.g., name, address, e-mail address (if an e-mail response has been requested). When a request has been fulfilled, we do not retain identifying information, with two exceptions--to add a user's name and address to a mailing list for receipt of a regularly issued publication, or to process for continuing education credits. Both are done solely at the request of the user.

Health Care Excel is the sole owner of the information collected on its site. Personal identifying information supplied by users will not be disclosed to a third party. When it is necessary to disclose sensitive information to serve your needs, we request that the user contact us.
 

Continuing Education Registration


Continuing education activities require registration before they can be used. Personal or other identifying information provided by continuing education participants through our web site may be disclosed only to professional licensure or certification boards upon written request for purposes of continuing education audits. Participants requesting their personal records must submit to HCE a written request and identifying information. Information collected from participants will not be sold, transmitted, or otherwise released except as necessary to process registration, obtain and/or verify continuing education credits, evaluate activities, and plan future educational events. The user may be requested to provide demographic information (such as income level and gender), and unique identifiers (such as license or certification number). Any information that is not necessary for the purposes of the registration will be optional.
 

Sharing


We share information on the use of our web site with some of our customers, such as the Center for Medicare & Medicaid Services, some state agencies, and others. We do not share personal identifiable information. Generally, reports are for verifying the level of page hits to our web site.

Health Care Excel is not responsible for the security, privacy, or content of any sites linked from this its site. When navigating to other web sites, users are encouraged to read privacy policies.
 

Security


Only HCE personnel who need the information to perform a specific task have been granted access to personal identifiable information. Furthermore, personnel are informed on security and privacy practices and what they can do to ensure that information is protected. Finally, the computer servers are kept in a secure environment.
 

Notification of Revisions


We will make a reasonable effort to notify users of any significant revisions to the privacy policy. Minor changes may occur that will not affect our use of individual identifiable information. When the privacy policies change that significantly affects the way we handle personal information, we will not use the information that was previously gathered without obtaining consent from the appropriate individual(s). We will post timely the privacy policy changes on our web site.
