QIO Congratulations
On November 29th, Health Care Excel hosted a Learning and Action Network (LAN) meeting in Plainfield, IN: Creating a vision to improve medication use. The event focused on the PSPC process and an interprofessional team approach to medication safety, to improve the delivery of care to patients specifically with anticoagulation, antipsychotics and diabetes medications.
Health Care Excel Shows Improvement in Care Transitions
In 2008, the Centers for Medicare & Medicaid Services selected Health Care Excel (HCE) as one of 14 Medicare Quality Improvement Organizations (QIOs) to participate in a three-year project that reduced 30-day rehospitalizations by 14.74% in the targeted community.
HCE of Kentucky Celebrates Successful Learning and Action Network Meeting
On March 20, 2012, Health Care Excel of Kentucky, the Medicare Quality Improvement Organization (QIO), hosted its second Learning and Action Network meeting at the Hyatt Regency in Lexington to serve providers and partners in the state. Cheri Lattimer, President/CEO of the Case Management Society of America and Executive Director of the National Transition of Care Coalition (NTOCC), delivered the keynote speech.
